2.Identify at least three specific organizations, groups or agencies that offer programs or policies in your county to improve the health condition or concern you identified. Hint: This may take some researching on your part–make a few phone calls, contact your discharge planner/social work department at your hospital, talk to someone at the community’s United Way offices or County Health Department.
