Having worked in the health and social care sector for the past 5 years you are now confident to make meaningful contributions to issues relating to public health in the health and social care institution where you work. As someone working in the health and social care sector it is important to ensure that you have a broad understanding of the role of public health and strategies for improving health at the community level. In order to do this the manager at your health care facility as well as the Public Health England (PHE) officer for your local authority have asked you to contribute to the annual public health report on the future agenda for improving health and social care. You are required to produce and deliver a written report on the role of public health in this improvement agenda. Your report should examine
the various approaches and strategies used to measure monitor and control the incidence of disease in communities; and the factors to be considered in the provision of community health and
social care services. The document should also outline the broad biological environmental and
social factors which can influence the health of individuals and communities.
Task 1:LO1: Understand different approaches and strategies used to measure monitor and control
the incidence of disease in communities.
1.1 Explain the roles of different agencies local national and international in identifying levels of health and disease in communities. In this section please be specific and support your explanation with examples.
1.2 Using reliable internet search engine (i.e. Google scholar) and textbooks and journals find statistics on the incidence and spread of infectious disease. After that you MUST explain the epidemiology of one infectious and one non-infectious disease and explain the relevance of statistic in the context of public health. (800 words)

2.2 Explain the relationship between the prevalence of disease and requirements of services to support individuals within the health and social care service provision.

social care settings. Produce a report on the factors which influence the health and well-being of the service users.
3.1 Assess the health and well being priorities for individuals in a particular health or social care
setting. By priorities this means determinants of health.
3.2 Evaluate the effectiveness of strategies systems and policies in a health or social care
setting.
3.3 Discuss changes that could be made to improve the health and well-being of individuals in a health and social care setting.
3.4. Evaluate any activity that has been implemented to encourage behaviour change for
maximizing health for individuals in a health or social care setting.(1000 words)
